The following error is generated when using storage vmotion to migrate a Windows 2012 VM on ESX4.0 
Relocate virtual machine A general system error occurred: Source detected that destination failed to resume. Migrating the active state of Virtual Machine

Resolution:

Manually copy the bios.440.rom file from the source LUN to the destination LUN straight after the migration has commenced.
